Brandeis University Waiver Requirement

 

 

The policy must be purchased through a U.S. based carrier company or U.S. based subsidiary.
Foreign insurance plans (including Embassy-sponsored plans) are NOT allowed by MA Regs.

  • Plans must provide comprehensive coverage including preventive and primary care services,
    ambulatory services, emergency services, and hospitalization benefits. These services must be
    reasonably accessible to the student in the area where the student attends school or the plan is
    not allowed by MA Regulations.
  • Out of state medicaid programs and MA limited plans such as Health Safety Net,
    Commonwealth Care (if < 19 years of age), and MASS HEALTH Limited, Basic and Essential
    are not allowed by MA Regulations (see note below).
  • The annual policy must have a minimum of $500,000 coverage.
  • Mental health and substance abuse must be covered as any other illness or injury.
  • Injuries or illnesses that occur as a result of alcohol, illegal drugs, or as the result of self harm
    must be covered as any other illness or injury.
  • Pregnancy and STI screening and treatment must be covered according to USPSTF guidelines.
  • There must be coverage for pre-existing conditions.
  • The maximum yearly deductable cannot exceed $ 250 (does not apply to family plans). Please
    review your network status, deductable, coinsurance and co-pay expenses against the cost of
    the QSHIP.
  • Emergency medical transportation, regardless of ER admission outcome must be subsidized.
