San Francisco State University Waiver Requirement

 

 

Effective Fall 2016, all F-1 International students are required to satisfy the insurance requirement through purchase of the CSU-sponsored insurance with Aetna through Wells Fargo Student Insurance Services at http://www.csuhealthlink.com.  There are two exceptions to this policy:

  1. Students on a government-sponsored scholarship which provides compatible insurance coverage as a part of the sponsorship.  To clear hold: E-mail a proof of your government scholarship that includes reference to your health insurance to OIP at oip@sfsu.edu.  Please include your full-name and SF State ID in your e-mail.
     
  2. Students receiving insurance coverage through an employer-sponsored plan (i.e. receipt of insurance is a part of the employment compensation through your spouse). 

    To clear hold: E-mail the following documents to oip@sfsu.edu: 1) A letter of your current insurance coverage from your insurance company, and 2) Proof of the  2016-2017 On Call International (Medical Evacuation & Repatriation) Plan (Purchase at http://www.csuhealthlink.com).  Please include your full-name and SF State ID in your e-mail.
